Athletics Clearance
All student-athletes must be cleared before participating in any Bosco camp, tryout, practice or competition.
Athletics Registration for the 2026-2027 seasons will be accepted electronically via AthleticClearance.com beginning May 15, 2026 (any physicals dated prior to May 15, 2026 will not be accepted).
STUDENT-ATHLETES WILL NOT BE PERMITTED TO PARTICIPATE IN ANY CAPACITY (CAMPS, TRYOUTS, PRACTICE, PLAY) UNTIL THIS ENTIRE PROCESS IS COMPLETED.
2026-2027 Athletics Registration Process
Student-athletes will not be allowed to participate in any capacity (camp, tryout, practice, play) until this process is completed. Please note that physicals must be performed by a physician (MD), doctor of osteopathic medicine (DO), nurse practitioner (NP), or physician assistant (PA).
Step 1: Register with AthleticClearance.com. All athletic clearance forms, including your son's preparticipation physical evaluation (i.e., the Bosco Medical Eligibility Form), must be completed and electronically submitted online through AthleticClearance.com. You must register with AthleticClearance.com, but there is no registration cost. Athletic Registration must be done each year before the start of school. Step 2: Download the Bosco Medical Eligibility Form here so that your doctor can fill it out. Keep in mind the following important information:
- All student-athletes are required to have an annual physical. The previous year's physicals will not be accepted.
- Physicals dated prior to May 15, 2026 will not be accepted.
- Physicals must be done by a physician (MD), doctor of osteopathic medicine (DO), nurse practitioner (NP), or physician assistant (PA). A chiropractor is not authorized to clear athletes for participation.
Step 3: Get your physical done and upload the completed Bosco Medical Eligibility Form to AthleticClearance.com. We do not accept hard copy forms.
Note: Bosco's Head Athletic Trainer, team doctors and staff will be hosting an on-campus opportunity to get your physical done on Saturday, June 20, from 8am-11am. Register and pay using the form below. Walk-ups will be accepted until 11am.
On-Campus Physicals
